The decisions you make concerning this sickness have repercussions that can be seen throughout the game, and it’s one of the more important choices you have to make in Assassin’s Creed Odyssey.

What is the Sickness in AC Odyssey?

On the southwest shore of the island of Kephallonia, you’ll find the village of Kausos. When you approach it, you’ll notice that it’s been burnt to the ground. The inhabitants are succumbing to the plague of unknown origin.

This sickness may just seem like a minor point in a small side quest, but it has more significant ramifications than are apparent at first. To start the side quest concerning the sickness in AC Odyssey, read on.

How to Start the Blood Fever Quest in Assassin’s Creed Odyssey

If you want to actually have any effect on the spread of the sickness, you’re going to have to complete the Blood Fever side quest. You can get this from Phoibe in Markos’s Vineyard before you leave Kephallonia for the first time. At some point after you go this quest will become unavailable, so make sure you do it before you leave the island.

The Blood Fever quest should become available after your third or fourth story mission. Keep checking your map, and you’ll find an icon marking where Phoibe is. When the mission is available, she’ll tell you about a curse on the village of Kausos and ask you to investigate. To do so, you’ll just need to head a bit southwest of the vineyard.

Should I Kill or Spare the Family in the Blood Fever Quest in AC Odyssey?

When you arrive at Kausos, you’ll find a priest and some soldiers are preparing to kill all the villagers. He tells you that this is to prevent the spread of the plague and is regrettable, but a necessity. The villagers beg you for mercy, and you’re given a choice.

If you choose, “Let these people go,” you’ll be forced into combat with the priest and his soldiers. After you kill them, you’ll get another choice. You can take the money they offer you or let them keep it. If you let them keep it, they’ll leave to find a cure, if you take it they stay in the village.

If you save the villagers and return to Kephallonia, you’ll find the whole island has been overrun by the plague. Furthermore, there’s a possibility (which remains unconfirmed) that if you let them keep their money, they’ll leave the island and spread the plague throughout the Greek world.

If you choose the option, “I have no say in this,” then the soldiers and priest will eradicate the village. This will eliminate the threat of sickness, and Kephallonia (and possibly the rest of Greece) will be saved.
