The Millicent questline is one of the major story threads in Elden Ring. However, finding both the quest’s starting point and Millicent’s locations after each part is hard enough that many players won’t make it to the end without calling on a guide. Fortunately, we’ve tracked down every place Millicent turns up and what happens when players get a chance to challenge or assist her at its conclusion.
Elden Ring Millicent questline: Locations and endings
Finding the beginning of the Millicent questline in Elden Ring is hard enough. It doesn’t actually start with her. Instead, players are asked by the sorcerer Gowry (living in the appropriately named Gowry’s Shack) to save a woman affected by Scarlet Rot.
Players will find Gowry’s Shack just north of Sellia Gateway and South of Sellia, Town of Sorcery. He’ll ask players to find an Unalloyed Gold Needle in the Swamp of Aeonia.
Players can find the Unalloyed Gold Needle as a reward for beating Commander O’Neil in the Swamp. After obtaining it, players should take it back to Gowry, and he’ll give them Sellia’s Secret and then ask for time to repair the needle.
Travel to a nearby Site of Grace and return to Gowry’s Shack to get the needle. Then, head into Sellia and light the braziers to lower the magical barriers.
First Location: Church of the Plague, Caelid
Head to the Church of the Plague, which can be found through one of the newly opened doorways in Sellia. Players will meet Millicent, who is not doing well, inside the church. They should give her the Unalloyed Gold Needle and then travel to a Site of Grace and return to the church.
After returning, Millicent will give players a talisman and say she feels the need to go on a quest. Then, return to Gowry and speak with him. After that, travel to a nearby Site of Grace and return to the shack to speak with Millicent once more.
Second Location: Erdtree-Gazing Hill, Altus Plateau
The next place players can visit Millicent is near the Erdtree-Gazing Hill Site of Grace at Altus Plateau, which can be reached by talking the Grand Lift of Dectus (or a secret side passage). After speaking with her, head to the nearby Shaded Castle and clear the area to find the Valkyrie’s Prosthesis.
Take the prosthesis to Millicent, and she’ll thank the player. After this, she’ll move on to the next place.
Third Location: Dominula, Windmill Village, Altus Plateau
In the north of Altus Plateau, players can find Dominula, Windmill Village. Players need to head to the northmost part of the area, beat the Godskin Disciple boss, and rest at the newly created Site of Grace.
After resting, players will find Millicent nearby. Just talk to her until she has nothing new to say, and she’ll move to the next area.
Fourth Location: Ancient Stone Valley Ruins, Mountaintop of the Giants
The next place players can find Millicent in Elden Ring is at the Ancient Snow Valley Ruins Site of Grace in Mountaintop of the Giants. Once again, speak to her until she has no more new lines.
Fifth Location: Player Room, Elphael, Brace of the Haligtree
This time, players will need to get the Haligtree Secret Medallion before reaching Millicent. She can be found in the Prayer Room at Elphael, Brace of the Haligtree. Speak to her until she repeats herself and then proceed downward toward the bottom of the Haligtree.
Sixth Location: Drainage Channel, Haligtree
Players will need to fight a Defiled Root Monster in the area immediately before the Drainage Channel Site of Grace in the Haligtree. This miniboss can seem tough, but players can cheese him by standing on the same branch they used to cross over to the area it spawns in and use ranged attacks to kill it with impunity.
Respawn at the Drainage Channel Site of Grace and return to where the Defiled Root Monster was to find a choice.
Elden Ring Millicent quest endings: Challenge or assist her?
There will be two summoning messages on the ground. One of them will have players assisting Millicent in the fight against her sisters, while the other has the player challenging her.
Assist her
(Note: An important key item is locked behind this choice.)
If players choose to assist Millicent, they’ll help her take on her four sisters. These ladies aren’t pushovers, and they’ll quickly kill high-level players. The key here is to run interference for Millicent. She’s a glass cannon, so players really just need to concentrate on keeping her sisters from ganging up on her. If players run interference, Millicent can quickly win this fight on her own.
Challenge her
Fighting Millicent isn’t too tough. She’s not got much in the way of HP, but she does hit hard. this is definitely the easier of the two fights.
Elden Ring Millicent quest rewards
For challenging Millicent, players will get the Millicent’s Prosthesis talisman which boosts dexterity and raises attack power with successive attacks. However, this is a bad ending, so players shouldn’t waste their time.
After assisting her, Millicent decides to die for some reason. She tells the player that she has already removed the needle and is waiting for Scarlet Rot to overtake her. Reload the area and return to find her dead and pick up the Unalloyed Gold Needle. Players can use this at Malenia’s flower (after defeating the boss) to get Miquella’s Needle, which is a very useful item if players accidentally get locked into a particular ending.
Unfortunately, we don’t get more insight as to who Millicent was. She’s one of Malenia’s kin, but we don’t know if she’s a daughter or if she’s some part of Malenia that split from her original form. Hopefully, we’ll find out more about her in the future and get some closure.
