You can manage to pick up the harmonica, which Sadie asks for in Chapter 3, before the fact. To do so, you’re going to need to head on over to Flatneck Station. From there, ride east until you come across Emmet’s farm (circled above).
While you may want to sneak around if you haven’t completed The Noblest of Man, and a Woman side mission, your main focus should be the largest cabin/building in the vicinity, with a campfire directly in front of it.
Walk into the cabin (for the avoidance of doubt, it’s the brown cabin with a grey door) and immediately turn towards the cabinet tucked in behind the door to your left. Use your Eagle Eye vision (click both sticks in) and you’ll see the red harmonica just sat there waiting for you. If Emmet is around, though, be careful: he’s liable to shoot you at any time.

Once he’s out of the picture, simply slide in and take the harmonica with no repercussions to speak of.
